What you will need the day of appointment:

• Valid Doctor’s order (this should have already been sent to us at least 48 hours in advance

• Photo Identification)

• Insurance Card or Payment in Full (we accept all major credit cards, cash and checks) if you are self-pay.

• Legal Guardian present if patient is a minor (under 18 years old)
